Admission Of Heamodialysis

Below is a simple flow chart showing how a new dialysis patient being admitted as an In-Patient for hemodialysis. 

Activity Responsibility Description Of Activity
    
Customer Service
  • Patient will register at the Customer Service Counter
  • Customer Service will direct patient to specialist clinic
  
Nurses 
  • Nurses will take over and new patient required to wait to be called by Nephrologist
  
Nephrologist
  • Counselling Session done by the Nephrologist
  • Date of commencement and place of dialysis will be decided by Nephrologist
  
Nurses
  • Nurse will fill up the Payment Form and Prescription (if in need of medication)
  • Nurse will direct the patient to Welfare Department to register as an In-Patient for Heamodialysis
  
Welfare Staff
  • Registration process will be done by our Welfare Department
  
Welfare Staff
  • Determine which category (eg. Government Servant / Non-Government)
  
Welfare Staff
  • Explain Dialysis Payment Mode, Request relevant documents, fill up documents, forms, claims, and so on will be done by our Gov. & Corp. Division Staff
  
Welfare Staff
  • Explain Dialysis Payment Mode Request relevant documents, fill up documents, forms and so on will be done by Subsidy Division Staff
  • Follow-up will be done for all applications

  • Eligible or ineligible will be stated in Welfare Coordinators Report
  • Eligible - will proceed to appy aid
  • Ineligible - Temporary close file Until further need arise

  • Compile All Document base on Kementerian Kesihatan Malaysia (KKM) requirements
  • In-Complete Applications will not be entertain by KKM
  • In-complete documents - follow-up
  
Welfare
Officer 
  • Documents complete will be submitted to the relevent Government / Non-Government Departments for financial aid
  • Follow-up will be done for all applications
